Thank you for entrusting the care and attention of your pet to Zasman Vet. This letter details our Practice Terms and Conditions. Some aspects of the Terms may not be relevant to you and we request that you ask for further explanation / clarification if required.

Fees
All fees, diets and drug charges are subject to VAT at the current rate and are payable at the time of purchase. Fee levels are determined by the time spent on a case and the drugs, materials, consumables and diets used. You will receive a detailed fee note for every consultation, surgical procedure or transaction with us. No drugs or food will be dispensed without payment. All appointments will be charged for unless an hours’ notice is given of cancellation. It is at the vet’s discretion not to charge for a consultation. The practice reserves the right to request a deposit of approximately 50% of the anticipated costs for non-routine surgical procedures. In cases where a patient is admitted for disease investigation, surgery and/or prolonged treatment, or where the client has no financial history with the Practice, the Practice may require some or all of the estimated fees in advance of starting treatment. With ongoing cases the Practice may require interim payments. We will endeavour to contact you with progress reports on both your pet and fees incurred. Please note that if a pet is registered with our practice, we will assume that any person other than the registered owner, who brings the pet in for treatment, is duly authorised by the registered pet owner to seek treatment for the pet and to incur costs for which the registered pet owner will be liable. Where a pet is not registered with our practice we will assume that the individual requesting treatment accepts liability for all costs incurred.
 
Methods of Payments
Accounts are due for settlement at the end of the consultation, the discharge of your pet or upon collection of drugs/diets. You may settle the account using:
* CASH
* CREDIT/DEBIT CARD – Switch, Solo, MasterCard, Visa, Delta (No American Express)
 
Estimates of Treatment Costs
We will happily provide a written estimate as to the probable costs of a course of treatment. Please bear in mind that any estimate given can only be approximate – often a pet’s illness will not follow a conventional course. It is the clients responsibility to request an estimate.
 
Settlement Terms
If you are unable to pay your account according to the standard terms of business, this must be discussed with the vet as soon as possible. Should an account not be settled then an invoice will be sent with additional Late Payment Charges, as well as Interest Charges of 8% plus Bank of England base rate, as per the Late Payment of Commercial Debts (Regulations) Act 2013. Any part payment will go to reduce the interest charge first. After due notice to you the client, overdue amounts will be referred to the county court and further charges will be levied in respect of costs incurred in collecting the debt. Please know that the use of third-party agencies and the county court could affect your future credit rating. Any cheque returned by our bank as unpaid, any Credit Card payment not honoured and any Cash tendered that is found to be counterfeit will result in the original amount being restored to the original sum with further charges added in respect of bank charges and admin costs, together with interest on the principal sum.
 
Pet Health Insurance
We at Zasman Vet strongly support the principle of insuring your pet against unexpected illness or accidents. Please ask for details about insurance from any member of staff. Please be aware that it is your responsibility to settle your account and then reclaim the fees from your insurance company. We endeavour to process your insurance claim within 2- 3 weeks. Please let us know if you need it done urgently as we can offer a Rapid Insurance Processing Service for a nominal fee.
 
Loyalty Card
The use of our loyalty card implies that you accept the Terms & Conditions as outlined below £10.00 minimum spend to earn one stamp.
One stamp per customer per day.
Rewards not valid in conjunction with any other offer.
No cash alternative available.
Not for re-sale.
Stamps from one Loyalty card cannot be transferred from one card to another.
  Stamps can only be collected on purchases made after 1st Sept 2012.
Purchases made from our online shop are exempt.
To claim your stamp you must produce a valid loyalty card at the time of purchase. Stamps cannot be awarded retrospectively.
Once you have collected twelve stamps you must surrender your card to a member of staff to receive the £10 credit to your account.
Zasman Vet reserves the right and has sole discretion to alter or amend any terms or conditions relating to this offer and loyalty scheme at any time.
Vaccine Reminders
We will endeavor to send you vaccine reminders, however we recommend that you keep note of when your pet is due their annual vaccination.  The post and computer problems could affect these reminders being reliable.  We do not accept responsibility for missed vaccinations.
 
Complaints and Standards
We hope that you never have recourse to complain about the standards of service received from Zasman Vet. However, if you feel there is something you wish to complain about, please direct your comments in the first instance to The Practice Manager in writing at 144 Liverpool Road, London, N1 1LA.
 
Ownership of Records
Case records including radiographs and similar documents are the property of and will be retained by Zasman Vet. Copies with a summary of the history will be passed on request to another veterinary surgeon taking over the case.
 
Ownership of Radiographs and Similar Records
The care given to your animal may involve making some specific investigations, for example taking radiographs or performing ultrasound scans, even though we make a charge for carrying out these investigations and interpreting their results, ownership of the resulting record, for example a radiograph, remains with the practice.
 
Prescriptions
Prescriptions are available from this Practice.  You may obtain Prescription Only Medicines, Category V, (POM V’s) from your veterinary surgeon OR ask for a prescription to obtain these medicines from another veterinary surgeon or a pharmacy.  Your veterinary surgeon may prescribe POM Vs only for animals under his or her care.  A prescription may not be appropriate if your animal is an in-patient or immediate treatment is necessary.  You will be informed, on request, of the price of any medicine that may be prescribed for your animal.
  We kindly request that you, the client, give us 24 hours notice for a repeat drug and food collection.  Any food or drugs advised will be charged for whether collected or not
 
Repeat Prescriptions
The general policy of this practice is to re-assess an animal requiring repeat prescriptions every 3 months, but this may vary with individual circumstances.
 
Disclaimer
No addition or variation of these conditions will bind the practice unless it is specifically agreed in writing and signed by one of the practice partners. No agent or person employed by, or under contract with, the practice has the authority to alter or vary these conditions in anyway.
